2010-2011 ENROLLMENT INFORMATION

Enrollment forms may only be picked up or returned on the following dates and times:
Tuesdays, Wednesdays and Thursdays from 9:00 - 12:00 and 2:00 - 4:00
(the office will be CLOSED for Spring Break from March 29 - April 9)

1.  You'll need to prove your residence before you can pick up an enrollment packet.  
  • 2 CURRENT utility bills
    • If not available, bring in the hook up receipts for the utilities OR 3 current items of business mail (insurance statements, credit card statements, doctor bills)
  • Your Driver's License / Govt. Picture ID - must match the name an address on the utility bills.


2.  Bring your proof of residence with you to the office and pick up your enrollment packet.

  • You can pick up your enrollment packets on the following dates and times ONLY
    • Tuesdays, Wednesdays and Thursdays from 9:00 - 12:00 and 2:00 - 4:00 (the office will be CLOSED for Spring Break from March 29 - April 9)
3. Fill out and turn in your enrollment packet.
  • You can turn in your enrollment packets on the following dates and times ONLY
    • Tuesdays, Wednesdays and Thursdays from 9:00 - 12:00 and 2:00 - 4:00 (the office will be CLOSED for Spring Break from March 29 - April 9)
  • You'll need to submit the following items with your enrollment packet:
    • ORIGINAL BIRTH CERTIFICATE, BAPTISMAL CERTIFICATE OR PASSPORT. CHILD MUST BE FIVE YEARS OLD ON OR BEFORE DECEMBER 2, 2010 IN ORDER TO ENROLL FOR KINDERGARTEN. If none of the above documents are available, a notarized statement will be accepted
    • IMMUNIZATION DATES – WRITTEN VERIFICATION IS REQUIRED for Kindergarten through 5th Graders
      • POLIO (PEDIARIX) 4 doses at any age or 3 doses if the last dose is after 4th birthday
      • DPT (PEDIARIX) 5 doses at any age or 4 doses if the last dose is after 4th birthday 
      • MMR (PROQUAD)(Measles, Mumps, Rubella) 2 doses, both after 1st birthday 
      • HEPATITIS B (COMVAX OR PEDIARIX) 3 doses required
      • VARICELLA (PROQUAD) 1 dose required or M.D. Proof of having the disease
      • MANTOUX PPD (TB SKIN TEST)
      • All kindergarteners and all new students from out of the state orcountry MUST have a TB skin test no earlier than 9/1/09 which is within 1 year of school entry) written documentation is required and must include date read and the results.
4.  YOUR CHILD IS OFFICIALLY ENROLLED WHEN ALL REQUIRED DOCUMENTS HAVE BEEN VERIFIED, SIGNED AND RECEIVED BY ONE OF OUR OFFICE STAFF.

ATTENTION: For Kindergarten and 1st Graders
A complete physical exam is required prior to the start of 1st grade. It is suggested to have the exam completed at the time the kindergarten immunizations are given. The date of the exam must be no earlier than 3/1/10 (which is 18 months prior to the start of 1st grade)
 

ORAL HEALTH ASSESSMENT (DENTAL CHECK-UP) California law, Education Code Section 49452.8, now requires that your child have an oral health assessment by May 31 in either Kindergarten or First Grade, whichever is his/her first year in public school.
